TRY-OUT ABSENTEE FORM

This form must be completed if a player is going to miss one or more of the required

player tryouts. This form must be completed and submitted with the player registration

forms in advance of tryouts.

All tryout and registration policies still apply, specifically:

• All registration forms and associated fees must be submitted prior to tryouts.

• Completion of this form does not guarantee placement on a team.

• If player is able to attend some tryout sessions, player placement procedure will

  be the same as for those attending all tryout sessions. Team placement will be

  posted on Arctic Foxes Website (www.arcticfoxes.org) and commitment dates

  will remain in effect. Player will be assigned a jersey color and number upon

  registration that will be used for this purpose.

• If player is not able to attend any tryout sessions, player will be evaluated by the

  placement committee via alternative method (i.e. stick time, RMU camp, 3 on 3,

  previous season player evaluation by coach). If player is offered placement on a

  team, he/she will have 48 hours to decline placement.
